Prospective Students

Required Documentation for Dow High School Enrollment:

  1. MPS School Personal Information Form School Entrance Form

  2. Completed Computer Use Guidelines Form Computer Use Form

  3. Student Records Release Form Release of Student Records Form

  4. Immunizations Records (As required by the State of Michigan) or
    Immunization Waiver (optional) Immunization Waiver

  5. Original Birth Certificate or certified copy
    We are required to examine the original or certified copy and make a photocopy of the form here. We are not able to accept photocopies.

  6. Proof of Residency (any one of the items below)
    * Purchase Agreement
    * Utility Bill (cable, water, consumers)
    * Rental or Lease Agreement
    * Property Tax Statement
    * Mortgage or Rent Receipt
    * Letter from Corporate Relocation Office

  7. Optional Items:
    * Language and Literacy Background Survey
    (only needed if English as a Second Language Services are requested)
    * Request for Special Education Placement Request for Special Education Placement
    (only needed if student is currently receiving Special Education Services)
    * New Student Athletic Transfer Information Athletics Form
    (only needed if student is transferring from another high school after their freshman year)
