Immunization Waiver (optional)

We are required to examine the original or certified copy and make a photocopy of the form here. We are not able to accept photocopies. -
Proof of Residency (any one of the items below)
* Purchase Agreement
* Utility Bill (cable, water, consumers)
* Rental or Lease Agreement
* Property Tax Statement
* Mortgage or Rent Receipt
* Letter from Corporate Relocation Office -
Optional Items:
* Language and Literacy Background Survey
(only needed if English as a Second Language Services are requested)
* Request for Special Education Placement
(only needed if student is currently receiving Special Education Services)
* New Student Athletic Transfer Information
